Preliminary Exam Delayed Conditions Form

PhD students should submit this form after completion of their preliminary delayed examination. The form will automatically populate a DocuSign workflow and send to all listed committee members. Routing order for the form is Out-of-Focus examiner, In-Focus examiner, then Faculty Advisor.

A copy of the completed form will be sent to the student, faculty advisor, and aerograd@colorado.edu.
