BCHM 4906 Internship Agreement and Learning Plan
This Agreement is made between the following Ƶ18 student (the “Student”) and the organization that is providing the internship experience (“Internship”) to the Student (the “Organization”). This Agreement does not create any obligations on the part of the Ƶ18 (“university”). The Academic Department, the Department of Biochemistry (“Department”), and the university’s Career Services office may utilize or reference this Agreement as necessary, including, but not limited to, for the approval of internship credit, compilation of statistics regarding university internships, evaluating insurance sponsorship, etc.
Completion of this Agreement is necessary to ensure a high-quality experience and satisfaction between the Student and the Organization. Completion of this Agreement may also be necessary for the Student to receive academic credit for the Internship if applicable. This Agreement includes an Internship Learning Plan to facilitate these goals. By signing this Agreement, Student and Organization acknowledge that they understand and agree to the responsibilities under this Agreement, including the Internship Learning Plan.
A copy of this Agreement should be completed by Student and Organization and will be automatically routed to Dr. Kristen Roy, internship coordinator in the department of Biochemistry.
The purpose of the Biochemistry internship program is to provide academic credit for volunteer experiences as well as paid internships in laboratory, clinical, or field settings. Students can earn 1 credit for every 45 hours they intern. A maximum of six internship credits may be accrued toward the minimum number of 30 credits needed for the BCHM major.
Check your Eligibility. To be eligible for BCHM 4906 Internship, you must:
- Have a cumulative GPA of at least 2.0
- Have completed at least BCHM 2700
NotetheFollowingConditions:
- Students will be charged regular tuition for internships.
- Students may not sign up retroactively for an internship that has already started or have been completed at an earlier time
Find an Internship:
- Students should search on Handshake for available internships. Partner companies may also post internship opportunities on the BCHM internship website.
- Please note: Internships must include an academic component that is related to the concepts the student has learned in the classroom.
- Contact an organization you are interested in to see if an internship position is available.
- Once you have found an interested organization, discuss and agree on roles and responsibilities, number of hours to work each week, and the total number of hours to work over the course of the internship. Develop a written plan to complete the agreed upon terms and responsibilities of the internship.

- Have an email exchange with the internship supervisor to confirm this information—save the email to include in your BCHM 4906 application.

- Important information before starting application:
- The department sponsor will be Dr. Kristen Roy (Kristen.roy@colorado.edu)
- You must be ready to include the internship supervisor email address (it is very important that all email addresses be correctly entered)
- You must be ready to attach the email exchange with your internship supervisor in which you have communicated with the supervisor the terms of the internship.

Conduct the Internship:
- Execute the internship according to the agreed upon terms of this application.
- Agree to work in professional manner and observe the required dress code.
- Maintain a digital weekly journal that records your day-to-day experiences.
- Keep a time log of the hours spent at the work site on a daily basis
- This log must be signed by the internship supervisor each week.
The internship grade will be based on the following elements:
- An evaluation from your internship supervisor; Approximately 10 days before the end of the internship the supervisor will receive an email containing the survey link from the department
- A written essay is required from the intern. This essay should be the equivalent of 2 pages per credit hour earned and include the following:
- Experiences and observations obtained with a record of days and times involved
- A careful analysis of the experience
- How the experience is related to the student’s major course work and career goals
- Times New Roman font and font size 12
- Upload the completed essay as a .doc file to the Canvas folder no later than one week after completion of the internship.
- Your completed weekly journals and supervisor signed time logs.
- Journal should be a single .doc file detailing daily internship experiences of at least ½ page per week at 1.5 line spacing with Times New Roman font and font size 12
- Upload to the Canvas folders a single file for each the completed weekly journals and the signed time logs no later than one week after completion of the internship.

Completing this form will pull up the application and agreement. After you complete the application and sign the agreement, that document will auto-send to the BCHM internship supervisor.The BCHM academic office will automatically receive notification when the DocuSign signature process is complete, and they will then enroll the student.
